Getting an ITIN number for book royalties from America

    I am selling an eBook on Smashwords web site in the US and have to get an ITIN number before I get paid.

    I am wondering if anyone knows anything about this. I have to fill in a form called a W-7.
    I have to accompany it with ID documents that I think are a passport and driving license.

    I have two questions:

    a) Will the American Embassy here certify my Passport and DL ?

    b) The W-7 form Instructions says:
    "Submittal of original/ certified copies of identifying documents for applicants using the combination of reason codes a and h is not required. Original documents and notarized copies will be accepted."
    [a+h is for "Nonresident alien required to get an ITIN to claim tax treaty benefit"]
    I am struggling to understand what this means. Do I have to get copies certified by the US Embassy or not ... ?

    c) I have to enter "the name of the treaty country and treaty article number" on the form. Does anyone know how to find this ?
