Need to get second learners permit

Hey, am getting serious case of tech rage from the ndsl website so please don't direct me back there.

    My provisional expires next week.

I have no idea if second provisional even exists anymore or what to bring with me in terms of documentation. I guess bring my old licence, fee, application form, passport and get my picture taken there.

Do I need original theory test cert (have no clue where it is or if I even have it)? Do I need a new "yes I need glasses" eye sight report? Where do I go? Back to Smithfield like where I got my first provisional? Should I book it or walk in?

    Thanks

What do you currently hold? A provisional license (which I thought they did away with years ago) or a learner's permit? If you hold a learner's permit, here's what you need to get a second one.

If you have an old provisional license, then I'd bring the proof of ID listed on this page just in case.

    http://www.ndls.ie/en/learner-permit/how-to-apply.html
Where you are renewing your learner permit, you must submit:
A fully completed NDLS "Application Form for a Learner Permit"
    The appropriate fee
    Your current learner permit
If applying for a third or subsequent learner permit you must submit evidence
    - that you have sat a driving test (Failure certificate) or
    - that you have a forthcoming driving test
A completed NDLS Medical report form if required in your case
    Evidence of PPSN
