How does the medicine course compare to the likes of RCSI, TCD or NUIG? (in all/any respects)

They are all completely different. They take a different slant on things and you can't hop from one to another. For instance, NUIG does lots of anatomy in first year but UCD doesn't do as much. There are other differences but at the end of the day, you end up with a degree in Medicine. It literally comes down to how you want to spend 5 to 7 years of your life. The opportunities are equal if you excel. Pedigree only becomes an issue if you are average / bottom of the barrel, in my opinion.
