Eye Test and Glasses Form Expired

    My Local Opticians applied for me to get an eye test and glasses form stamped from the HSE and the HSE sent me the form abck so I could go and get a free eye test and glasses from the opticians.

    I Thought the expiration date for the form was 2015 but that was the expiration of the medical card. Apparently the form had to be used within 60 days which has just passed as the form is stamped 16 June. What can I do? Do I have to wait another 2 years to get an eye test and glasses or can I just apply again? Any chance they'll accept this form still?

    I reckon you will have to apply again, but it only takes 2/3 weeks usually.

    No way should you have to wait another 2years from date of that form.
