Insurance claim-template for description of events

I received a dental injury over the weekend at my local sports club and, as I am hoping to a make claim through the insurance policy, I have been asked to provide a description of events. Where would I get a template with all the different questions to be answered in order to provide all of the required information?

Why would you need a template? Just write down what happened.

Insurance company should have a claim form available for you to complete.
