They Lost my Garda Vetting Form

I don't want to state the company or body i question but I do a job where I work with children. I obviously have to be Garda Vetted.

When I did my first course in this job I had to fill one out. I gave it to the person running the course, as we were instructed to and off I went. I did the job all throughout 2012 and then found out via out union noone had been Garda Vetted for 2012 as the forms were misplaced.

As you can imagine the **** hit the fan. We were all told to re do forms. I personally handed mine in addressed to the child protection officer in the company.

I did my refreshers course for the job that year and was told I HAD TO fill out another despite doing so already only a few weeks and personally handing it in.

I think at a meeting of workers had the new Child Protectio office come in, read names out who werent vetted (mine was there despite already giving in
    2)

I filled one out in front of this gentleman and gave it to him like many did.

At my refreshers course in 2013 and again in 2014 I was told to fill out another one and 2 weeks ago I got an email saying I was cleared to do my job as they never got a form off me.

This is a company that is huge in this country, that directly involves people working with kids left right and centre and Im disgusted at this and wondering what should I do next

Data protection commissioner as they keep "losing" your personal data.

are you an employee or are you a student assigned to the organisation. It is not clear from your post as you mention refresher course.

My understanding is that you will need to fill out a form for each assignment. So if you are assigned to the organisation for a period in a number of different years then you will have to fill out a form each time.

Have you raised this with the child protection officer? And what did they say?

Should there be a "not" in the above sentence?

I'm not a student no, I am an employee. I would rather not use the body I actually am involved with but lets say I was a football coach. Working for a club who pay me and I work with children, training and managing them.

I'd fill out one of these forms and give it to my clubs child protection officer who would then look after in in regards getting me vetted or sending it onto the FAI. Only he looses it. And I get a letter from the FAI stating I can't coach as Im not vetted. And lets say this happened 4 times in 2 years, for the exact same position with the same club they continuially askling me to fill these forms out as they had lost them.

    Taking your FAI example
1: Fill in the Garda Vetting form again and photocopy it before giving it to anyone
2: Send the completed form to the relevant person in your local organisation along with a covering letter telling them that is the x time in y years that you have completed and returned the form to them. Tell him that you are very disappointed that the Club keeping losing this information and hope that this x copy of the form will not be lost.
3: Send a copy of this letter to Chairman and Secretary of the Club
4: Send a copy of this letter to the Child Protection officer, CEO etc of the National body.

Also write out a receipt, hand your form directly to the child protection officer and have then sign your receipt.
