Going for my class 2 medical (initial) and was wondering if anyone remembers the format of the form?

I looked up the CAA and FAA forms which you can get a pdf of online and the main difference appears to be a section where you have to write appointments for the last 3 years. I would have to think about that and would have no idea of dates etc. Is that the case with the IAA one? If you disclose appointments would you have to get a letter from the doctor? eg. there is coeliac disease in our family so we were all tested about 2 years ago including an out patient procedure but it came back negative, would you need to disclose that? Surgery 30 years ago? I'd say that doctor has bitten the dust if I need a letter ;)

If you're on medication (not disqualifying) do you have to bring a letter from the prescribing doctor?

Just don't want to go to be told to come back with supporting documentation.

That would be a "precautionary" test - there maybe a better word for it

Bit like if you went for a diabetes test every now and again
