Power of Attorney Questions

Just editing this to put two questions into one thread.

My parent has dementia and I'm a little late on catching up on some important things. I don't want to be naive and I don't want to overstep the mark either.

I'm trying to work out two things: whether my parent set up an enduring power of attorney at the time of making their will and secondly, if they didn't, if there is any way to find out if a family member had this registered close to the time of my parent developing dementia?
Is there a register of sorts that any family member can access either by themselves or through a solicitor to find out if this was done by another family member?

I also don't know what solicitor my parent chose to make their will with.

Also, do you have to set up any form of power of attorney when you make a will and do you have to nominate someone as your executor or can the solicitor be the sole executor?

I've searched a bit online and can't find anything useful on this!

    Appreciate any help.
