Implantologist Advice/Recommendation

    Hi,

    Looking for advice well a recommendation on one or a couple dentists/dental surgeons to go to with regard to an implant.


    So here is the back ground first - missing top front tooth left as you would look at me from checking online believe it is number 9.

    Had an accident approx 16 years ago which impacted the front of my face and broke my tooth in the gum and slightly shattered/broke the bone around it. Some of the bone at this point had to be removed. Its was originally bridged but then removed within a year and have been wearing a denture for the last 15 or so years.

    I am looking at getting an implant and realise that I will need a bone graft if possible at all. My dentist in the past has told me that it is feasible but have never took it any further as he said he would have to refer me to someone else to do.

    I dont want to end up going to someone who is not at the top of their game as it is obviously one of the most visable teeth/things that is noticed when speaking to anyone.

    I have seen online the likes of clinics advertising in Ireland for 1350 which they say is a half price offer (you know who they are) which is a right bit cheaper than their next closest. And I have read up quiet a few posts and reading up on the costs of the kit involved so am envisaging a price in and around 2400-2600 including the bone graph.

    Also when the bone graft is done would I be wearing the denture until the gum heals? then the screw?


    I have no interest in going abroad to get any work done and will not be going up the north either. Ideally around the Leinster area would be best. So if you can PM me any recommendations it would be gratefully appreciated.


    I have seen "Oral Surgeon" + "fitzgeme" give some good advice so if you could chip in.

    That sure is a long winded post..............

    Lots of options here....

    If the tooth suffered a trauma and had been gone 15 years then yes you are most likely looking at a bone graft, maybe a gum graft too...

    An assessment must first be made of the bone height and width. Despite the trauma and the time missing, generally the site can be worked with as the adjacent teeth "hold" the bone height somewhat and the palatal bone holds quite well, the bone on the lip side is long gone I'm sure...
    In my experience of these, only some patients will need a bone graft alone, leave for 6 months and then later place the implant.
    Most patients can have the implant placed with simultaneous bone grafting if the remaining bone if fair... Angulated implants are available to engage more of the resielient palatal bone and in doing so reduce the amount of bone graft needed.
    Some patients can get away with no graft but I would say that this is not you.

    Gum grafting may be needed if you show gum when you smile as it is the gum symmetry that matters... You think you are looking at the teeth but really it's the teeth and how they emerge from the gums... If you have a low lip line and show no gum or have decent bone and gum already, you may not need this... I take gum graft from the palate and graft it into the site....

    I don't like dentures sitting on a healing surgical sites. I prefer to fit a bonded temporary bridge to the adjacent teeth during the healing periods. It's far cleaner....

    Don't be lured in by foreign treatment. There is only a few big jobs involved but a good operator will have you back for loads of little check visits as needed...

    Ask to see the clinicians work at the consultation. Good clinicians take good photos of their good work.....
