Paramedic/EMT flat rate expenses Revenue

Kind of surprised to discover there is no Flat Rate Expenses amount allowed for in the Revenue guidelines for ambulance crews. Most medical type professions seem to have one, why none for Paramedics? Anyone know why this is the case.
