It's a walker TV not that it matters, and there is a good quality outside antenna

Is she using an indoor or outdoor aerial?
Is there any type of amplifier, masthead or distribution, in the aerial setup?
How far is Cairn Hill away?

A 100% signal strength with 0% quality could indicate too much signal.

    Hi

It's an outside aerial and no amp, not sure how far it is maybe 20-30 miles,

Mux 1 from there is perfect,

The existing outdoor aerial should be ok but may need to be realigned to optimise the best signal for both muxes.

When did you try the scan?

Something odd happened to Channel 44 Cairn Hill on Good Friday during Late Late show, signal dropped to almost nothing then returned to full strength but no RTE 1 or RTE 1 +1.

I did a manual rescan on Channel 44, got both TV channels back as normal. No problems since.

    Hope this helps.

From the reading you give is that the signal from the aerial is too powerful and you lost your signal quality.
Get a plug in attenuator to reduce your signal strength and that should get your signal quality back up.
Ideal the signal quality should be 100% but the signal strength can be from 50% upward.
Your signal strength may be more than 100% but the TV just show 100%
