NDLS Eyesight report form

    Hi, I am going to apply for my learner license over the coming days and I was wondering if I can use my recent eye test information to complete the eyesight report form or will I have to do another eyetest ?

    I got my eyesight tested in specsavers for new glasses about 1 month ago.

    I don't think a recent eye test will suffice. A specific eye exam is done for the drivers license application. After the exam, they print off a specific form & you bring that in with you when you apply for your license. When you go in for an eye exam for glasses, they don't give you any paperwork. They may give you a print out with your glasses prescription, but that's about it. Back when you were doing your recent test for glasses, it is possible that you could have done the two tests at the same time, as long at the optician knew at the time what you needed. But as that was a month ago, its probably too late now.

    This form must be submitted to National Driver Licence Service with an application for a driving licence/learner permit
    within one month of its completion by a Medical Practitioner or Optometrist.

    I have the form printed out and ready to be filled in. What I was wondering was will I need to do another eye test for this application ? Prouddub says I probably will have to.
    Anyone else sure about whether or not my prescription will suffice ?

    Nope. Your prescription will not suffice. That is just a series of numbers that sets out how strong the lens in your glasses need to be, so that the people who make the actual lens know what to make. The folks in the NDLS centres are not opticians. They are not trained to read opticians shorthand, or know what the various prescriptions mean. The required eye exam report gives a detailed report of your eye sight, in lay mans language. That is what the NDLS centre needs.

    It can be filled out by hand, by an optician. But the most common way to do it, is by computer. Opticians have a template of the forms requirements on a computer. When the exam is over, they fill it out on the computer and they then print it out for you. That report is what you need to bring into the NDLS centre. Just bringing in your glasses prescription won't cut the mustard.

    Hey,

    The test is a normal eyesight exam, with the addition of a peripheral vision test - flashing bars on a screen and a buzzer for you to press.

    Most doctors can also do eye test if you happen to be going to the doctor for anything.

    perfect thanks, might be able to get it done for free in college. I'm curious to know whether they actually check them or not..logistically it seems quite difficult

    Get you to read the chart and look at your eyes with light.

    Thats all I had to do.

    Sorry to piggyback this thread, but I was just wondering if anyone could tell me how much Specsavers charge for the eyesight report you need for applying for a learners permit?

    Sometimes it's €15 or €30 check their site or even give a call it use to be €15 when on offer.

    I'm confused. Do I need a medical report as well as the eyesight test. I always wear glasses and have scheduled a test at Supersavers, but the instructions imply I need a doctor's report as well.

    For certain age (older) and truck or bus type licence you woyluld need a medical.

    Ndls.ie will have all info you need.

    Yes, but NDLS has confused me. I don't need a medical by reason of age or the class of vehicle. I am just, truthfully, declaring that I need to wear corrective lenses. Their info isn't clear.
    My eyesight isn't so bad, I am a bit short sighted and last year the optician told me I just fail the criteria.

    No, you don't need a medical report. You only need one, if you suffer from one of the medical conditions listed on the NDLS website, or you are on medication, that could affect your driving ability. If you do, you need to get a medical report from your doctor. If you don't, all you need is the eyesight exam.

    Just got this reply from NDLS:
    An eyesight report is only required if you are a first time applicant or if you used wear glasses for driving but are now no longer required to do so. If you wear glasses and it hadn't previously been noted on your licence then you will not be required to provide an eyesight report, you would just need to tick yes to the question regarding glasses/corrective lenses as a declaration to add a 01 notation to your licence to show that you need to wear corrective lenses at all times whilst driving.

    They told me I didn't need the report when I went in. I had one done before hand anyway.

    If this is your first time ever applying for a learner permit or if your last learner permit expired more than 5 years ago then you will need a D502 eyesight report, no ifs, ands or buts.

    Did a bit of ringing around and just for other's benefit, Bill Dawson in Rathfarnham Shopping Centre are doing the test for €20. Got it done within an hour of calling.

    I don't know then, I asked them in the store and they told me that you can't book an appointment and that I need to pay 10€, thats it.

    Ok so to clear things up, when a driver applies for there first learner permit then an eye sight report is mandatory. The reason for this is because of the notation that gets put on the back of a license/permit. If the eye sight report said you need glasses you will have a 01 notation, if it said you do not you will not have a 01 notation.

    Now lets take a driver who needs to wear glasses for driving, they would have a 01 notation on there license. When applying for a renewal unless there has been any change in eye sight then an eye sight report is not needed.

    For example if said driver got lazer eye surgery and no longer wears glasses for driving then an eye sight report in needed to remove the 01 notation form the license/permit. This would be the same for drivers who never wore glasses but now do, then an eye sight report in needed to add the 01 notation.

    Now with regards to medical reports, on the medical report form there is also a question regarding eyesight. So a medical report will always trump an eye sight report form, there is no need for both.

    The medical report is only required if you answer yes to any of the medical questions on the application form or if you are applying for any of the category two vehicles such as trucks or buses.
