If I don't return the medical card review form to the HSE, how likely is it that my card will be withdrawn due to them not getting the form back?

There's no likely about it - it will be withdrawn. The paperwork with the form will have made this explicitly clear if they haven't changed it.

Thanks for your quick reply. The paperwork does say that, so do you think that they will adhere to that threat?

100% they will. That's the entire point of the review - find people who's circumstances have changed, have died without being reported, etc and remove them if the form is not completed and with satisfactory information on it.

It isn't a threat it is a promise. They have a target to reduce the number of medical cards in circulation and this is the easiest way to do it. There will always be a percentage who don't reply for whatever reason.
You won't hear from them again but when you go to use your card you will be told it isn't valid despite still being in date. By the time you notice you've been struck off and do apply and are assessed the HSE will have had several months of not having to pay your GP.

Its automatic. If no review, then the system automatically cancels card.
