questionnaire re accident

    i am current out of work on sick leave due to having an accident on my way home from work.

    i received a questionnaire today about the accident, i need to show them proof of my hospital visits but they dont ask what my injuries are?

    i am just wondering what the point of this is and will a copy of invoice from A&E and a copy of my appointment card suffice?

    Who did you receive the questionnaire from and what was the reason for receiving same?

    it was from the department of social welfare. it didnt say what the reason was, i am assuming it is something to do with my sick pay which i have been receiving for the last number of weeks.

    You can be paid Occupational Injury Benefit or Illness Benefit. The questionnaire is probably just to assess if you qualify for Injury Benefit which potentially could mean an extra 6 months payment in addition to Illness Benefit.

    Thanks. I sent it off during the week. My maternity benefit will kick in next month anyway.
