Tooth abcess

    Just looking for some advice.

Had root canal treatment with a crown last year. Paid 1400 in total for both.

Had excruciating pain radiating from the tooth last weekend. Went to dentist on Monday did an X-ray and I have an abcess in the root, ie root canal failed.
Gave my options as 1) refer to an endodontist to get a 2nd root canal dobe 2) oral surgeon to get tooth extracted and implant put in. 3) a bridge but really didn't recommend this option as I'm only in my 20's.

I was told that as it would be a re root canal there's only a 70 % success rate so I decided that the extraction and implant is probably best....all before I heard how much it would cost...3400 euro!

Dentist gave me anti biotic and anti inflammatory and made an appt to see the oral surgeon who works in the practice every fortnight.

24 hrs later I ended up in A and E with the pain wairibg until 5am to see a doctor who gave me a few solpodol. I returned to see oral surgeon who gave me a second anti biotic and script for painkillers. She talked me through the extraction which is scheduled for 2 weeks time.

That was yesterday and the pain is still there. I'm exhausted from it aswell as vomiting from all the meds I'm taking.

Can anyone advise? Is the extraction and implant the best option? When should I expect the antibiotics to have effect....ie pain to subside? Is the 3400 for extraction, bone graft, implant and crown a reasonable quote?

Get a second and third opinion on the price. Can you go back to the original dentist that did the root canal and ask them to complete it?

Thanks for your reply. It is the original dentist, it wasn't that the root cabal was uncompleted just unsuccessful which is apparently always a risk. I've been unlucky but my savings are going to take a serious hit as a result!

Who did your original RC, was it an endodontist?

If a root canal has not been successful then a second root canal should be proposed but indeed has a lower success rate as they have advised.

Now, the best thing to do is remove the tooth, you're in pain so it needs to go... 2 weeks is a long time to wait in pain just to suit the dentist...
The tooth needs to be removed in an minimally traumatic fashion to preserve as much of your bone as possible in advance of implant placement... So don't just let anyone do it...
Waiting in pain and possibly infection is doing your bone no favours...
Are they planning to remove the tooth and place a bone graft in the socket that day??

Immediate post extraction bone graft into an extraction site with an acute infection? Surely not.

As far as I'm aware the extraction and graft were due to be done on the same day. OS can you recommend an oral surgeon to get a 2nd opinion with? I'm really confused with the whole thing and scared if how much money this is going to cost me.

It was a general dentist the first time around not an endodontist. I really don't want to go down the route of a second root canal with a chance it may not work at all.

I have to get it sorted ASAP, very difficult period in work at the moment and just can't be in this much pain trying to do my job!

Exactly! Thought the bill from last year was bad enough without this!

You're describing it well but I don't know the full facts but...
A symptomatic root canal treated tooth is only symptomatic because it is fractured or infected. Both would probably be a little too grubby in my opinion to do an extraction and socket graft same day, but that's just me...
I actually have turned away from socket grafts in most situations. If the site is excellent post extraction, an implant can often be placed straight away. If the site is not ideal, I now favour removing the tooth very carefully and allowing the site to heal for 8-12 weeks and then place the implant....
I don't like graft anymore if it can be avoided.....

If you post your location, I can pm a recommendation close to you for an opinion...
