    Hey guys, hope this is ok to ask here.

    I've got paper certs for two sets of shares, and I'm hoping to send them off to my broker now to get them dematerialized.

    Never had to fill out the form though before. It's on the back of the share certs, so want to make sure I get them right. So a few quick questions...

    Is it ok to leave "Number of Ordinary Shares to be sold" blank, or should I put "0", and on a similar note, do I just fill out the number of shares I have currently in the "Number of Ordinary Shares to be kept" section?

    Then, there's a section which says "Full name of the person to whom the security is transferred". Would I just put "AIB" there, or their full name, or do I need a specific branch, name, etc?


    And finally, do I just leave the "Date", "Participant ID" and "Member Account ID" sections blank? Presume that's for something on their end?

    Sorry to be a nusiance, just want to get this form right :)

    All I can say is contact your broker, but maybe others will be able to help.
