Wordpress Form with Attachment

Hi All,

Can anyone recommend a plugin or alternative to expand the normal contact or submit form on a wordpress site to include the option of attaching a document (any file type) and sending it to an email account?

No bulk upload required, single document per user preferably with no login requirement. I'm working on a couple of options but nothing is working as intended.

If you don't know how to directly edit template files, try this...
