    But wasn't the BA one a few months back due to a maintenance error with the cowlings not being properly secured after routine maintenance rather than a bad design of the cowlings themselves ?

    Apparently the cowling latches are at the very bottom of the engine, and it's very easy to miss the fact that they aren't secured/closed properly.

    The latches will look closed but can still be open.
    Cfm latches hinge down. It's a dual inspection item so either 2 engineers missed it or just didn't bother to check.
