Housing needs asssesment form

    I just received rent allowance and I got a letter the other day requesting I get a houseing need application assessed and once the reply is received to forward it on to the rent unit.

    Has anyone got one of these forms before?
    I found this one http://www.sdcc.ie/sites/default/files/form/housing-needs-assessment-form-2013.pdf
    But there's no mention of a postal address. Any ideas? :)
