Child with different surname Travelling to America

    I am travelling to Florida in a couple of weeks with my 2 year old daughter.
    Her father is not travelling with us. I was wondering what sort of documents I would need to bring with me as on her passport she has her dads surname.

    Do I need to go to a solicitor or is there a simple form I can get signed so I am not stopped in Orlando airport with a child that doesn't have my surname.

    If both your names are on the birth cert, that would seem one important thing.

    I checked wit the dept. of foreign affairs only last week as I had the same query, i was told its no problem once tickets match passport and esta's are done before travel
