What car and engine is it? Diesel?

Only thing I can think of is that the idle is off for the test or it won't rev high enough.

Swap out the lamda sensor id imagine?

Lamda is giving the wrong o2 levels which is making the revs unstable and ruining emissions.

Get a diagnostic cable connected and check the lamda.
