    Yes you do need to bring a form in with when your getting you eyes tested, Bring it into the eye opticians or Doctors where ever your getting your eyes tested because they need to fill out one part and to sign it and you also have to write your signature on it while in the present of a doctor or optician. You can get the forum in your local tax office or download it from the RSA site.

    When I got mine I asked when booking it if I needed to bring one but they said it was ok as they had them.

    If you are making an appointment ask them do you need one. If you are just walking in then bring one with you to be on the safe side.

    Also remember you may be able to claim the cost of the eye test back on your PRSI.
