I cannot enlarge the .jpg but it looks like stricture of the oesophagus - Carcinoma. Could be 'malnutrition and exhaustion'
Basically this is a cancerous growth on the tube that connects the mouth to the stomach. At about 75% blockage food gets stuck, you cannot/do not want to eat. With chemo and today's medicine the five year survival rate is about 50%. In 1903 you said your prayers.

Looks like "stricture of the oesophagus, carcinoma 6 months, malnutrition & exhaustion, Certified" to me.

Nearly +1. Inanition instead of malnutrition though.
