Problem with tiles

I have big tiles laid in our kitchen living room area which have been there for approximate 2.5 years. The floor underneath is joists +underfloor heating + 2 layers of plyboard. Not long after being laid the grout started to crack and lift and some of the tiles came loose. The tiler came back and relaid a few tiles and regrouted an area but after a short while the problem came back and since then it has got progressively worse. Now I have quite a few cracked and loose tiles and everyday I'm sweeping up grout. I've kind of given up on the original tiler and figure my problem is down to one of two things :
    1. Too much give in the plyboard below resulting in the tiles working loose
    2. The underfloor heating affecting the tiles -maybe the adhesive used wasn't right or the heat is cracking the tiles ( I heard this can happen but am not convinced)
    Or a combination of both.

Either way I suspect I'm looking at having to have the tiles lifted and relaid but unless i know the route cause of the problem i'll be back to square one pretty quickly.

So would like any advice available..

    Many thanks
