Applying for Medical Card

Hi I am a 25 year old mature student and I am curious as to how I may apply for a medical card. I haven't been 'on the books' employed in the past 3 years, just doing a bit of part time bar work at weekends and rent has been subsidised by my parents. I co-habit with my girlfriend.

Can anybody advise me as to what steps I may need to take and documentation that I will need to apply for a medical card? Any advice would be really helpful? I have no real proof of income, as the job pays cash in hand, have never received any 3rd level grants or signed on for the dole etc. As for outgoings well mostly it's just food, rent, broadband and costs for commuting at weekends.

    Simply apply online.
If you are between 16 and 25, dependent on your parents, really, and in school, you will not have to show your income, but will have to prove being in school.

I have completed the online form but I am not sure if the details I have provided are correct. I am just curious to know what has to be physically sent with the form I received via email that has to be completed by hand.
