Med 2 form query

My wife had some dental work (Crowns) done recently and as she doesn't work I paid for the treatment.
From what I understand I can claim back 20% of the amount on my tax if I submit a Med 2 form .
Do I ask for the form to be made out in my name or hers or does it matter ?
    Many thanks

Its in her name, but if you do a joint return you can make the claim.
