I need to go intohospitalfor a day proceedure
Nothing serious but has to be done.
I need to fill out the request form for a day off but do I have to take it as a holiday or is it sick leave?
I am not actually sick - it is just a test.
I am not quite a full year there and am not permanent yet.
    I would appreciate your advice.

Sick leave is at the companies discresion in most cases, read your employee handbook and contract.

I know in my place, I wasn't permanent until I was there 6 months.. and until I was permanent, sick leave was unpaid.

    Just ask I suppose!

Take it as leave. Sick leaves are not looked upon favourably by most companies (this is often unofficial)

Only your HR person can tell you.

But if I had to guess... if you were in the company for a while and had developed your relationship with HR, you could probably take it as a sick day. But you're new so will probably have to take it as a day off.
