    Last week I received the form JSP B in the post.

    This is the second time in the last 2/3 months I've received this form to fill in and bring into the office at an allocated time.

    I was just wondering does anyone know why I've received this, especially twice in such a short amount of time.

    They have all this information from when I originally signed on and the info is the same as when I last filled in the form a couple of months ago.
