Allowance for employing a carer

My father has employed a carer and I was trying to figure out how to fill in the form HK1 for him. I have set him up as an employer.

It says on the revenue website that you can either fill it in at the end of the year or on an ongoing basis.

When filling it in on an ongoing basis, it says that his tax credits would be adjusted. Do I just estimate the total costs for the year (or does the refund work based on last year)?

Also, on the form, it has 4 spaces for the net cost. That means the max is EUR 9,999. Is this cost per month? The limit per year is 50k, so that isn't enough spaces.
