    Should I have received a UCI number or should I leave it blank?

    Does the IMM1295E require an electronic signature or should I sign it, scan and upload it? The only problem is the Validation button obviously wont work if I print + scan it. Any help?

    Also is the IMM5707E the correct form or should the IMM5645 be filled out instead?

    Last question, does the payment have to be made by credit card/bank transfer?
