Form for car tax got ruined

I was off for a few months working away from home and I got the form filled out and stamped by the guards but the form got wet. There was information specific to my car on the form. What can I do now regarding getting a new form? Thanks very much :)

You can get one from your local Garda stn. motor tax office, or download from motortaxie
