    *SYNOPSIS *

    The co-owner of G-MYGH, having completed a number of circuits on Runway (RWY)
    30, handed over the aircraft to the Pilot who intended carrying out one solo
    circuit. While landing on RWY 30, the aircraft’s right main wheel caught in
    an electrified wire fence, leading to a heavy landing on the runway and
    damage to the aircraft. The Pilot switched off the electrics and fuel and
    exited the aircraft safely.

    An examination of the passenger list revealed that there were a number of airline staff on board The Cabin Crew identified that one of these was a Boeing 767 captain from another airline with 11,000 hours flying experience.     Out of curiosity, how would this work? Did they look at the list, spot a name that the Captain recognised and pop down to see if he was the person they thought? Or do airline staff, even for other companies, declare it when booking?

    Airline staff would be travelling on special ticket code IDX0 or ZED.

    The passenger manifest (thats the long dot matrix print out) certainly has extra info after certain passengers names (i.e frequent flyer status) so anyone an IDX0/ZED is going to be flagged there, what exactly is on the list is airline dependent

    Typically company pilots or those from another carrier getting a free ride would identify themselves to the senior cabin crew onboarding

    Interesting, is that the same as non-rev pax? I often fly non-rev with work as the majority of my work is for airlines, but I don't work for an airline, so would I marked as ZED/IDX0??

    That F/O was released from hospital that afternoon and flew home to FRA on the evening LH flight.
    I remember thinking he can't have been that sick?

    It's impossible to know from the report but I'm sure you can imagine that if it had, for example, been a severe migraine he would have been incapable of flying the plane and yet it would be safe for him to be released to travel home.

    The report did mention that he was met at Frankfurt by a medical service to bring him for further medical examination so it less "he's grand now" and more "he's stable enough to be brought closer to home".

    SYNOPSIS

    The Instructor and his Student were conducting a cross-country training flight with an en-route landing at EICK. Following a touch-and-go landing on RWY 17 at EICK the engine lost power at a low height. The Instructor assumed control, banked left and landed the aircraft in a grassy area adjacent to the end of the runway. After a short ground run the aircraft collided at low speed with the airport’s chain link boundary fence. Although the aircraft was damaged there were no injuries.

    The Investigation did not conclusively determine the cause of the engine power loss although conditions were such as to make carburettor icing a likely cause.

    The guidance given in regard to a forced landing due to an engine failure after take-off is to limit the landing sector to the 60° sector ahead (30° to the left and right). In this case that was a poor option due to the nature of the terrain ahead and consequently the Instructor made a large turn to the left. Most importantly, he had sufficient height and maintained enough airspeed to be able to achieve a steep 115° turn without losing control of the aircraft by stalling. To a large part this was due to good handling by the Instructor who quickly lowered the nose of aircraft when engine power was lost, thus maintaining the aircraft’s energy level and airspeed until touchdown.

    The aircraft was departing from Brittas House Airfield in Co. Limerick on a private flight to a
    destination in the United Kingdom. With normal take-off power applied and the aircraft well
    established in the take-off run, the Pilot felt that the aircraft was not accelerating as
    expected. Despite the application of extra power the speed did not increase sufficiently for
    the take-off to be continued safely. The Pilot attempted to overfly the deep ditch at the
    departure end of the runway but made contact with fencing posts in the process. The
    aircraft came to rest at the far boundary hedge of the adjacent field and sustained
    substantial damage. The Pilot exited the aircraft without injury.

    During a Search and Rescue (SAR) mission in the vicinity of Hook Head, Co. Wexford on the afternoon of 6 December 2015, a Sikorsky S-92A helicopter crew located and winched two casualties from the water. During the winching operation one of the casualties, a 14 year old girl, slipped from the rescue strop and fell into the water from a height of approximately 45 ft. The casualty was recovered immediately from the water and winched up to the helicopter again. Both casualties were transferred to a nearby hospital in Waterford for medical treatment. The Investigation was subsequently informed by An Garda Síochána that one of the casualties, a 14 year old girl, had died on 10 December 2015.
