I get some meds spam, and penis enhancement spam (I'm female) but it goes directly to junk mail - I use Microsoft Outlook and it does a very accurate job of filtering. If I label something spam its then permanently spam and goes to junk mail.

In my 'last week' junk mail I have 31 items of which 5 are pills and penis ads. In the same week all items in my inbox were legitimate mail.

Maybe try setting up a new gmail account and pull the e-mails from the old account into it as gmail run a pretty good spam filter. My old hotmail and yahoo accounts started to get bad, did this and was never bothered by spam again. I currently receive a grand total of zero spam e-mails. Most of my accounts are quite old at this stage as well, the gmail bordering on 6-7 years, the hotmail and yahoo accounts are both over 10 years.

My Yahoo a/c is 16 years old, tbf it does a good job of keeping the Spam out of my main inbox and into the Spam folder!

I get very little spam to my gmail address (and all of it goes into the spam folder), but my hotmail address gets tonnes of it (understandably- it's my "junk" address that I use to sign up to things).

I get stuff about pills, being the biggest my girlfriend has ever had, fake match.com, Christian dating, Jew-date (I'm in a long-term relationship and have never been on a dating site, btw), fake online schools, and, even though I only worked in Portugal for a MONTH four or five years ago, I'm always getting ones offering to "aumentar o meu pênis". I don't seem to get any girl-specific spam.
