Over 70s medical card

    My father is applying for an over 70s medical card and his spouse is entitled to the same benifits under the same card.
    The thing is they both attend different doctors in the same locality
    My question is can they get the card with two doctors names on it or do they have to attend the same doctor
    I don't think either want to change
    Any advice appreciated. Thanks

    Get another over 70s application form. Tear off the last page which pertains to the GP of choice. Have your mums GP complete this page. just clip this page to the already completed application form, so you have a page completed by each GP. Make sure you have included all the documentary evidence as requested, and that each GP has actually signed and stamped the acceptance form. Good luck.
