Transition Pension

Am currently on pre retirement payment from social welfare. as I turn 65 next year I assume I will transfer to transition pension payment. Could anyone tell me, 1 Do I have to apply anywhere or will payment happen automatically? 2 As my wife turns 65 three months before me will I lose any of her payment?
