Fill out the RF100A form and get it stamped.

Download and print this (Or get it from the Garda station or Tax Office) and fill in relevant details and dates, but don't sign it

Go to Garda station, sign it on front of a Garda who will stamp it (Tell him why it's been off the road and for how long)

Bring it to tax office and tax car

    Simple

Garda is just witnessing your declaration, not certifying it as true or correct. If you've got a slew of parking tickets or speeding fines during the time you're declaring it off the road, they could use it as evidence to charge you with a making a false declaration.

Very true. OP should be aware that being "off the road" is literally that, the car must not have been driven or even parked on a road or other public place at any time.

    There's a confussions there.

According to motor tax legislations, car must be taxed if it's being used on public roads (being parked on public road is obviously invluded).

But on declaration on RF100a form, they want you to declare, that car was not used in public place.

Hence public road is not the same as public place. (every public road is public place, but not every public place is public road).

So there is a chance, (unlikely but there is), that someone didn't use his car on public roads, but did use it on public place (f.e. it was parked in public place, which is not public road).
In that case, legally there was no need for the car to be taxed, but motor tax office would require owner to declare that car was not used in public place, which would be not true.
Therefore it is impossible to avoid paying tax for period when car was used in public place which is not public road, without making a false declaration.
