You can't be *certain* no one else has access. Change your password asap.

one of your friend's picks up a virus somewhere that farms their contact list then spams the contact list pretending to be someone else from the same list.

Or, you picked up a virus or had a webpage open sometime when you also had your email open and your contact list got slurped. Its not difficult to fake the sender of an email.

changing your password might be an idea just to be on the safe side. Delete all temporary internet files (make note of passwords you will need again) and run AV + anti-spyware/malware on your system.

Try to use different passwords on different sites to avoid compromise from an external source (just because google stores your password securely doesnt mean the website you sign up to with the email account as your contact or username has a similar security setup).

Get into the habit of changing passwords at irregular intervals (Easter this year, Christmas next year, birthday the year after). As magic Ron says, you cant be certain no-one else has access but you can make it as unlikely as possible.

oh, and if its an online email account, check your sent items. if unknown emails are sittign in there then they are definitely from that account :) if they arent in there, then it means you cant be certain but its less likely.
