As I say, I'm self-employed, is it possible for me to claim relief for my spouse's non-routine dental procedures ?
The form I have seems to be geared for PAYE claimants only,

You claim it on your form 11 for that year.

If its within the 4-year period you have not missed the boat .......But its Med 2 and not Med 1 .

You should get your Dentist to sign this form , as non-routine procedures could be anything and some of it may not be claimable .
