I find it rather annoying that "distros" are released which are only cosmetically changed something else.

It is akin to taking your Ford car, giving it a paint job and calling it something other than a Ford ..... or something 'based on' a Ford.

There are real distros out there, some of which are truly forked from something else, and are standing on their own ..... and could survive if the 'based on'/'forked from' distro disappeared.

Most of what are presently called "distros" could not survive in such circumstances.
IMO they do not deserve to be called 'distro'.

I think each distro has its own slightly changed Kernel, but yeah, mostly cosmetic changes is what is seen on each distros

Even Ford did it with the Fiesta / Mazda back in the 90's. Same production line.

+1 if it totally reliant on a base distro and isn't at least handing back large chunks of code then it's not really a distro

though I'd make an exception for something like Knoppix / puppylinux, as they've got a long history and serve a specific purpose rather than being yet another a 'me too' skin

Agreed. If something like Lubuntu counts as a separate distro, then distros like Gentoo and Arch are a different distro pretty much every time you install one.

Is it possible to distribute something that is not constant?

I think I'll release Philosofix, based on a virtual machine with no host and hidden files that are created and deleted with no resultant effect on their environment.
