Per the great big diagram above, you discuss it by private message with the moderator in question. If you can't arrive at an amicable resolution, you start a thread here containing enough information for a category moderator to make a useful start on figuring out what's going on. Information such as the forum from which you were banned, the reason you were given, stuff like that.
