What is a SC4 Form?

    Hi,
    So i have just compleated a 16 week cource with Fas which included two week's work experience. at the end of the work experience the manager of the shop asked me how did i like the work experience and i replied positivley and he then asked me if i would like a full time job and i said yes definitely:D. so i have a meeting next week to sort out hours and all that stuff.

    well on the last day of the fas course we were all given a letter saying that a sc4 form will be posted to your house

    so im just wondering what this form is about and if it will mean anything to me as i will be starting work and wont be singing back on the dole ?
