Changing location on medical card

    How do i go about changing location with the medical card. Are there any forms to fill out. Can these be got online?

    If you move house
    You can use your medical card for up to 3 months if you are living temporarily outside your Local Health Office area. In this case, you can attend any GP in the area participating in the medical card scheme. If you are going to be away longer than 3 months, you should apply to the Local Health Office of that area, for a medical card. If you move to a different part of your own Local Health Office area, you can apply to change your doctor.


    Your new GP will have the forms req.

    This is the form that you will need. https://www.sspcrs.ie/portal/medapp/changedoctor.pdf
    You need to fill in the details and get your new GP to sign and stamp it for you. Post to PCRS (address on form). However even thought the delay in issuing medical cards has been improved substantially I find that transfers are still extremely slow and I find many taking far more than 3 months. Your new GP may still see you under your old medical card but different surgeries have different approaches on this so check with them.
