Study Plan?

I'm trying to study and in other threads people say "stick to your study plan" but I have no clue how to make one. I studied like twice in 2nd year and that's it, I'm clueless when it comes to studying :o I didn't do great in the mocks so I really need to study hard. I don't know where to start, I'm so confused :(

Fill in everything you do during the week including school, time with friends, homework, meals and of course study. Follow it everyday of every week between now and your JC.
