Need Help with Childcare Supervision module in Fetac 6

Hi All - I'm completing Advanced Supervision in Childcare Fetac 6 through distance learning and could do with some advice on the assignment.

When they say complete a Learner Record in Childcare Supervision with 6 different questions, should this be laid out like an essay with subheadings?
Also the first question asks us to reflect on an action or initiative taken in the workplace by me. Could somebody please give me an example of what they mean? Like is it an action taken in play with the children or an action taken as Supervisor of the team. I guess its the latter but I'm just not sure. I'd appreciate any feedback anyone can offer! Thanks for reading!

I have finished the course around 2 weeks ago and this is how we were advices to do the learner record:

We were given a list of possible titles like parental involvement/partnership , kolbs supervision model , team meetings , supervision contract, reflective practitioner this is some that I can remember but if you need inspirations I will look at my notes.

The way each title is laid out is like an essay so factual like explain what it is using references then you would write about an experience you had like say if went to a team meeting in your work experience then you would reflect on it saying if it was effective and were problems solved.

In general reflection means that you think about the experience you had seeing it or not in the work experience setting.
