Form 11 - shares

    Last year the company I worked for was sold and I received shares.
    I paid a tax - the company helped us to calculate and fill in the form.
    I was also told that this year I have to submit Form 11. The person who helped us with tax told we just need to fill in the part for the shares (and details from P60 in the appropriate boxes).
    I have now downloaded that form and it looks very complex to me.

    There are parts for savings account(s), rent, pension and medical insurance – do I need to fill in all of that as well?
    I am paying the rent and have tax credit on that.

    You only have to complete those parts of the return that apply to you.

    Ignore anything else.

    If you are claiming a particular credit - rent, for example - you must puit those details onto the return, or you will lose the credit.

    Thank you so much for your reply. I really appreciate any help on this.


    Would you be able to help me with few more questions about the form:


    1. Rent - I have moved apartment last year. The section 530 (Rent Tax Credit) contains only one box, should I just put both rent amounts and landlord details in the "free" space beside?


    2. Medical Insurance - I was insured only from May to November, so should I just calculate the actual amount the employer has paid (not annual cost of the insurance)?


    3. Pension - What is the correct box for the pension 221 or 225?


    4. Savings Accounts - I had savings account in the bank which has been closed (Anglo Irish Bank). Is it possible to get the statement from them about the interest I have received? Unfortunately I don't have any statements, I didn't know I might need them for anything once the money has been transferred.

    Would a Form 12 be any use in this situation, or is that for something completely different?

    No, I was told it is Form 11 I have to submit and the reason is I have received shares last year.
