my front derailleur problem ?

    Hi all,

    cycling home today and the chain got caught in the front derailleur so i turn pedals backward and it free'd its self but then when i was pedalling the chain was hitting off the front derailleur so i got home and tried to adjust it with this video.

    i have got it back to 99% normal again just sometimes when i am shifting to big cog on front it skips over and falls onto crank. And when i am on the big cog at the front and big to medium cog on rear it hits against the front derailleur. would appreciate it if you could tell me what to do to fix this, is it a simple fix?

    Angular adjustment of FD relative to seat tube (to avoid chain rub) and mechanical stops screw adjustment (stops chain from coming off during gear change). I use the park tools procedure.
