I think you are wasting your time filling out a CAO form unless you start learning how to spell.

To answer your question, all the information you need is given to you in the CAO handbook. Have a read through, and if you don't understand something, ask about that specifically. The sticky at the top of the forum also answers some of the most common things that people tend to get confused with.
