Just nod. It could be awkward if you enter a room and shake hands with everyone, then look the them and just nod.

I'd just be a little worried about foot cheese although I guess if the foot was being extended then it's owner would be vigilant about that type of thing.

Unless of course they got some sort of buzz out of smearing their foot-cheese on people.

See that's where things get complicated. I'd have to check for foot-cheese first, especially if it looked like contagious foot-cheese, nobody wants a dose of herpes of the toe.

That being said we do tend to shake hands straight away without looking, don't we? Sure that hand could have been anywhere, up a nose, up a hoop. Maybe we're being footist. I don't know Chuck, you've given me a lot to think about.
