Personally I wouldn't get tired of getting pampering breaks/treats or breaks away :)
For Mum, there's a lovely inexpensive necklace I got for my Mum one year that's a nice touch, it's a little nest with eggs in it, one for each child she has had. There's lots of different types http://www.etsy.com/search/handmade?q=bird%26%2339%3Bs+nest+necklace&order=most_relevant&ship_to=IE&view_type=gallery

My Mum loved it, and then my sister who has kids really like it too, so she got that for her birthday :).

No need to go really expensive, honestly for women I think the thought counts a lot more than the price.

It depends on what they're into, and if you're ruling out the majority of present ideas it makes it tougher to suggest things because it becomes a lot more personal.

Is there something perhaps from your girlfriends' childhood that you could get as part gift? or something that means a lot to her / from a point in her life?

Is there something that she'd enjoy doing? like horse riding or dirt biking or lessons for dancing / skill perhaps?

Yes it's hard to give you ideas when we don't know what she's interested in. One thing that occured to me is how you said she had a tough year, that suggested to me that maybe something that would make the next year more pleasurable for her would be a good thing. So maybe something like a course of treatments, or yearly membership of a club/theatre/concert venue, or for example these magasine subscriptions that last a year or two, see the idea ? something that would last more than just the Christmas.

I came across a site that did a sort of "this is your life"book, you could put whatever you like into pictures, ticket stubs etc, memories and things like that, you send everything to them and they will make it up for you in a really nice looking leather book. I thought it was a really nice idea, but can't remember the site.

Girlfriend of over 10 years, eh? Some jewelry perhaps - a ring maybe - you could go all out as it's Christmas and make it a diamond ring. Present it to her while tying your shoe.

Either that or the latest steam iron.

Not sure what they're all in to, but here are some ideas:

Into reading - kindle, great little gadget, got one myself recently
Into computers - nifty little netbook or a tablet IPAD 3 if you can afford it or you could much cheaper ones like an Archos
Into music - nice MP3 player perhaps
Clothes - a day and night in a hotel in Cork/Galway/Dublin/ Limerick and a nice shopping spree followed by a nice meal in a nice hotel
    Guitar sounds good as well.
