    hi im looking to get my baby a passport, myself and her father are not together and our baby has both our surnames so i think on the form it says a guard has to witness both of us signing the form and they stamp the form and photos. is that it then do we all need to have our birth certs with us to for the guards to see them too or is that just for the passport office.. thanks
